Patient Portal
Once we schedule an appointment to meet, I will send you a link to a secure online patient portal. This portal is encrypted, and the information is only accessible to me. I ask that you please complete some brief forms, prior to your first visit.
If you are already a patient, you can log in to the portal here:
The portal can be used to upload private documents, access billing records, and check the status of your appointments. You can also opt not to use the portal after you complete the forms.
